Health Home Coordinator
Penquis, a social services agency serving Penobscot, Piscataquis and Knox counties for over 50 years, is recruiting for a Health Home Coordinator in its Family Enrichment Services (FES) Department. This position will be based in the Dover-Foxcroft and Bangor areas dependent on needs of the program and requires travel. This is a part-time position with the potential of full-time. The Health Home Coordinator will provide care coordination/case management to adults who have an approved Mental Health Diagnosis. In the BHHO model, you will participate in a team-based approach to care, by working in collaboration with your supervisor, Peer Support specialist, clinician, Nurse Care Manager, Psychiatric, and Medical Consultants. As a Health Home Coordinator, you will work with individuals to address behavioral health needs, as well as integrating physical health/medical needs. You will work with a diverse population. Candidate should possess excellent communication, interpersonal skills, documentation skills and the ability to multitask.
Requires a current MHRT certification. Previous experience working with individuals with mental health issues or case management experience preferred. Experience with MS Word and Excel required. Must have reliable transportation and valid State of Maine driver’s license in good standing, and reliable, insured transportation. Upon hire and periodically thereafter, must have background check results that are satisfactory to Penquis and indicate no previous or current record of involvement with child abuse, neglect or exploitation and no disqualifying criminal record or motor vehicle record as outlined in current contract provisions.
