Care Coordinator/Case Management
Penquis, a social services agency serving Penobscot, Piscataquis and Knox counties for over 50 years, is recruiting for a part time (24 hours/week; benefits eligible) with potential for full time Health Home Coordinator in its Family Enrichment Services Department. This position will be based in the Dover-Foxcroft and Bangor areas depending on the needs of the program and will require travel through these areas.

The Health Home Coordinator will provide care coordination/case management to children who have a DHHS approved mental health or intellectual disabilities diagnosis. In the BHHO model, you will participate in a team-based approach to care, by working in collaboration with a Clinical Team Leader, Family/Youth Support, Nurse Care Manager, Psychiatric and Medical Consultants. As a Health Home Coordinator, you will work with children and their families to address behavioral health needs, as well as integrating physical health and medical needs. You will work with a diverse population of families. Candidate should possess excellent communication, interpersonal skills, documentation skills and the ability to multitask.
Requires a Bachelorโs Degree in a Social Work, Child Development, Human Services, or related field. Previous experience working with children or adolescents with disabilities or case management experience preferred. Experience with MS Word and Excel required. Must have a valid State of Maine driver’s license in good standing, and reliable, insured transportation.
The starting rate of pay for the Health Home Coordinator position is $19.55 per hour.ย Penquis offers paid training, mileage reimbursement and an excellent benefits package.
