Join our Team! Penquis, a social services agency serving Penobscot, Piscataquis, and Knox counties for over 50 years, is currently seeking a part time (24hrs) with the potential to work into full time, Health Home Coordinator.  As the Health Home Coordinator, youwill provide care coordination/case management to children who have a DHHS approved mental health or intellectual disabilities diagnosis. This position is remote, with travel and in-person meetings required in the Piscataquis County area.   

Location: Dover Foxcroft, Maine

Health Home Coordinator Responsibilities:

  • Work with individuals to address behavioral health needs, as well as integrating physical health and medical needs.
  • Participate in a team-based approach to care, by working in collaboration with a Clinical Team Leader, Family/Youth Support, Nurse Care Manager, and Psychiatric and Medical Consultants.
  • Work with a diverse population of families.
  • Work with children and their families to address behavioral health needs, as well as integrating physical health and medical needs.

Health Home Coordinator Requirements:

  • Bachelor’s Degree in Social Work, Child Development, Human Services, or a related field.
  • Previous experience working with children or adolescents with disabilities or case management preferred.
  • Must have a valid State of Maine driver’s license in good standing and reliable insured transportation and available to work flexible hours to meet the needs of the client and families.
  • Possess excellent communication, interpersonal skills, documentation skills and the ability to multitask.
  • Upon hire and periodically thereafter, must comply with Penquis background check requirements with satisfactory results as outlined in current contract provisions.

The rate of pay for this position starts at $20.95 per hour, and additional compensation may be given consideration with relevant experience.
