Penquis, a social services agency serving Penobscot, Piscataquis and Knox counties for over 50 years, has an opening for a part-time Health Home Coordinator; potential to become full-time. This position will be based in Bangor, Maine serving Penobscot and Piscataquis counties; travel required.
The Health Home Coordinator will provide care coordination/case management to adults who have an approved Mental Health Diagnosis. In the BHHO model, you will participate in a team-based approach to care, by working in collaboration with your supervisor, Peer Support specialist, Clinician, Nurse Care Manager, Psychiatric, and Medical Consultants. As a Health Home Coordinator, you will work with individuals to address behavioral health needs, as well as integrating physical health/medical needs.
Must have a current MHRT/C certification. Previous experience working with individuals with mental health diagnosis or case management experience preferred. Experience with MS Word and Excel required. Candidate should possess excellent communication, interpersonal skills, documentation skills and the ability to multitask. Must have reliable insured transportation and valid State of Maine driver’s license in good standing.
The starting rate of pay for the Health Home Coordinator position is $19.26 per hour and additional compensation may be given consideration with relevant experience. Sign on bonus, up to $ 3000 for April, May, and June !
