Health Home Coordinator (Children) – Penobscot/Piscataquis County

Job Overview

Penquis is seeking a full-time Health Home Coordinator to provide care coordination and case management to children with approved mental health diagnoses. This role combines direct client support, team collaboration, and travel throughout Penobscot and Piscataquis counties.

Responsibilities

• Address behavioral health needs while integrating physical health and medical needs into care planning.
• Work collaboratively with a team that may include a supervisor, Peer Support Specialist, Clinician, Nurse Care Manager, and psychiatric and medical consultants.
• Complete accurate clinical and annual assessments.
• Track MaineCare review dates, assessment reviews, and treatment plan due dates.
• Support crisis intervention and follow-up services, including the development and implementation of crisis plans.

Requirements

• Bachelor's degree in Social Work, Child Development, Human Services, or a related field.
• 2 to 3 years of experience working with children, adolescents, or adults with disabilities.
• Valid State of Maine driver's license in good standing with reliable insured transportation and availability to work flexible hours for client and family needs.
• Ability to meet Penquis background-check requirements upon hire and periodically thereafter, with satisfactory results as required by current contract provisions.
