Behavioral Health Home Organization (BHHO)

Behavioral Health Home Organization provides individualized treatment plans for children and adults based on the client's personal needs. These can include basic, mental and behavioral health and medical needs as well as housing, education, vocation, finance, socialization, and development of natural peer supports.

Behavioral Health Home Organization (BHHO) is a collaborative, client-focused service offered in the home and the community, helping individuals obtain long-term recovery.

Services include

• An individualized treatment plan based on the client's personal needs, including basic needs, mental health needs, and medical needs
• Connection with doctors and other support providers in the client's life
• Help getting social services, transportation, referrals, and other supports
• Support aimed at reducing emergency room visits or hospital stays
• A team-building approach to care, with the Health Home Coordinator working closely with a nurse, clinician, medical provider, and peer support specialist to best meet each client's needs

Eligibility

For adults:

• 18 years of age or older, with a mental health diagnosis and MaineCare

For children:

• 2 to 21 years of age, with a DHHS-approved mental health or intellectual disability diagnosis and MaineCare
